APPLICANT AGREEMENT
1. I certify that the statements and information furnished by me in this application are true and correct and understand that falsification of such statement or information will result in withdrawal of the employment offer, or immediate dismissal.
2. I authorize investigation of my personal and employment background, including an FBI background check, BCA background check, local courts check, credit check and DMV check. If hired, checks will be done annually.
3. I authorize investigation of all statements contained herein. I also authorize the references listed above and on my Personal History Statement to provide to the Lower Sioux Indian Community Businesses all information concerning my previous employment and any pertinent information they may have, and release all parties from liability for any damage that may result from furnishing same to you.
4. In consideration of my employment, I agree to conform to the rules, regulations and policies of the Lower Sioux Indian Community Business.
5. I realize I cannot be employed by Jackpot Junction Casino Hotel if convicted of a felony or while employed. I understand and agree that, if offered employment, I will be required to show evidence that I am either a U.S. Citizen or a legally employable alien.
6. I understand and agree that, if hired, my employment is for no definite period and may, regardless of the date of payment of my wages and salary, be terminated at any time without
prior notice and without cause.
7. In accepting employment, the employee is agreeing to be available for any hours scheduled. Schedules are subject to change at any time.
8. I recognize that all records, documents, materials, lists, drawings, books, programs and all other property of the Company made or received by me are property of the Company exclusively.
9. I understand that before I can be formally hired by any Lower Sioux Indian Community-owned business, I will be required to take a test to screen for the presence of drugs and/or alcohol.
By submitting this application, I consent to take that test when requested. I understand that if the test result is positive:
a) I will not be hired;
b) I will not be considered for any other jobs in any other Lower Sioux Indian Community-owned business for a period of 12 months.
c) The test results will be entered in a database, accessible to all Lower Sioux Indian Community-owned businesses for a period of two years.
